The type of personal information I collect

The personal information that you are asked to provide, and the reasons why you are asked to provide it, will be made clear to you at the point I ask you to provide your personal information.

  • Emergency contact

  • Case/session notes
  • Health information (only relevant to the issue you come to me for)
  • GP details (I will only contact with consent and if required)

  • All information gathered (with your consent) is for the sole use for your appointment(s) and records kept by me for a minimum of 8 years in accordance to professional guidelines set out by the National Council for Hypnotherapy (NCH) of which I am a member of.

    You have the right to request to see these at any time.

How I use your information and why I have it

I use the information I collect in various ways, including:

  • For me to keep accurate records of my client's for HMRC purposes, which include invoicing
  • So I can refer back to our previous work should you return as a future client (this can help us both to plan your treatment).
  • To provide evidence of clients I have worked with to my accrediting body. This entails your name, email address, the dates and number of hours that we worked together. They also have privacy policies in place. They are the QCHPA and NCH.
  • If in the event I wish to send you newsletters and information concerning workshops, talks and events that I offer, you will need to opt in and sign up on my website. You may unsubscribe or be asked to be removed at any time.

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:
  • Your consent. You are able to remove your consent at any time. You can do this by contacting us using the details in the footer of this website.
  • I have a contractual obligation.
  • I have a legal obligation.
  • I have a ligitimate interest.

Providing your personal data to others

We may disclose your personal data to our insurers and/or professional advisers insofar as reasonably necessary for the purpose of obtaining and maintaining insurance coverage, managing risks, obtaining professional advice and managing legal disputes.

Financial transactions relating to my website and services may be handled by payment providers such as SumUp. I will only share transaction data with my payment service providers to the extent necessary for the purposes of processing your payments, refunding such payments and dealin with complaints and queries relating to such payments and refunds. In addition to the specific disclosures of personal data set out above, I may also disclose your personal data where such disclosure is necessary for compliance with a legal obligation to which I am subject, or in order to protect your vital interests or the vital interests of another person.

The hosting facilities for my website are situated in the UK.

Data retention

I am bound by the code of ethics of the National Council for Hypnotherapy (NCH) of which I am a member of and also my insurance to hold onto records for a minimum of 8 years after our last session together. For children under the age of 16 this will be when they have turned 25. For children of 17 years of age, I am bound to hold the records until they reach 26 years of age.

Cookies

Under the data protection law/GDPR, you have rights including.
